NSW Health in Parkes and Forbes invites applications for a Nurse Practitioner–Hospital in the Home role. The HiTH NP leads the Lachlan Health Service HiTH team, working with ED, inpatient and community services to deliver patient-centred care and prevent unnecessary hospital presentations.

Role involves identifying eligible patients, coordinating referrals, and working across Parkes and Forbes sites. Shifts are 08:00–16:30, Monday to Friday, with permanent full-time employment and a salary

Responsabilidades

• HiTH NP leads the Lachlan Health Service HiTH team, coordinating referrals from the Emergency Department, Inpatient Unit, Aged Care Facilities and Community Nurse service to HiTH to prevent admission to hospital or ED visits where possible.
• Works closely with the Forbes HiTH RN and other staff to identify appropriate patients for referral and admission to HiTH.
• Shifts 08:00 to 16:30, Monday to Friday.

About the role
This position is in Parkes; however, it is considered to be a Lachlan Health Service position. Lachlan Health Service incorporates both Parkes and Forbes, and staff work across both sites when required.

As a HiTH NP you will join a team of expert clinicians who provide patient centred care. The HiTH NP leads the Lachlan Health Service HiTH service, working closely with the Forbes HiTH RN, other nursing and medical staff to identify appropriate patients from the Emergency Department, Inpatient Unit, Aged Care Facilities and Community Nurse service for referral and admission to HiTH to prevent admission to hospital or presentation to the ED where possible.

Shifts 08:00 to 16:30, Monday to Friday.
