• Registered Nurse seeking Nurse Practitioner Candidate (NPC) role.
• Clinical leadership and expertise to develop, implement and deliver integrated care for older persons.
• Ability to provide professional and clinical support across service streams.

Responsibilities

• Provides clinical decision making and advice via telephone triage to RACF clinicians, supported by Emergency Physician and/or Geriatrician.
• Expert acute / emergency gerontic clinical assessment and care planning and linkage to appropriate services.
• Communicates with hospitals, RACFs, GPs, residents and community groups to ensure right care, right place.
• Works to achieve high quality patient outcomes and report against key performance indicators.
