Job summary

Gold Coast Health is seeking a Nurse Practitioner to join the Acute Care Team on the Gold Coast. The role focuses on planning, implementing and evaluating integrated care to optimise outcomes for referred consumers.

You will provide advanced nursing interventions and collaborate with the care team to ensure seamless service delivery within our modern facilities.

Qualifications

• Bachelor of Nursing or equivalent required.
• Masters of Nursing (Nurse Practitioner) or recognised equivalent.
• Registration with the Nursing Midwifery Board of Australia is required.
• Credentialing in Mental Health Nursing or working towards credential is preferred.

Responsibilities

• Provide advanced nursing interventions including initiation, titration or cessation of medicines.
• Apply teaching strategies to convey diagnostic information effectively.
• Collaborate with the health care team to achieve optimal consumer outcomes.
• Drive system improvement and reduce fragmentation and barriers to care.
• Coordinate and perform clinical assessment/intervention for individual consumers.

The Role

An opportunity has become available for a Nurse Practitioner (After Care Support Service) to join Gold Coast Health (GCH) where the focus of our 14 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

• Provide advanced nursing interventions that may involve initiation, titration or cessation of medicines
• Apply appropriate teaching/learning strategies to provide diagnostic information that is relevant and theory based
• Collaborate and consult with other members of the health care team to achieve optimal outcomes with the consumer
• Facilitate system improvement by actively enhancing system/partnership integration and reducing fragmentation and barriers
• Provide point of contact coordination and clinical assessment/intervention for individual consumers

About you

To be successful in this position you will require:

• A Bachelor of Nursing or equivalent
• Registration with the Nursing Midwifery Board of Australia
• A Masters of Nursing (Nurse Practitioner) or recognised equivalent
• Be credentialed through the College of Mental Health Nurses, or working towards a credential in Mental health nursing
• Driver Licence
• Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
• Tuberculosis screening, to identify if the tuberculosis vaccination may be required
• Declaration of serious discipline history must be disclosed
• Criminal History Screening will be required for new employees (GCH will cover cost)

• Base salary ranging from $79.85 - $83.37 per hour

The Acute Care Treatment Team (ACT) provide 24-hour, 7-day a week specialist mental health care services across three operational teams. The Gold Coast University Hospital (GCUH) / Intake team's role is to be the first point of contact for the Gold Coast Hospital and Health Service Mental Health Service to provide triage, assessment, education, support, information, advice and referral. ACT Community is a mobile outreach crisis response team that offers a range of short-term options tailored to meet individual needs in the community which may also include acute care in the immediate post discharge phase of an inpatient admission. ACT is committed to providing a service that is empathic, courteous and able to develop positive relationships with people in crisis, founded on hope and recovery.
