Job summary

LDK Seniors Living is seeking a Registered Nurse – Care Partner at Amberfield, Canberra. You will bring assessment skills, clinical judgement and coordination to your own portfolio of residents, guiding their care journey with a holistic approach.

Work closely with residents, families and multidisciplinary teams to personalise care, develop care plans, and support wellbeing. The role emphasizes dementia care, continuous professional development and alignment with LDK’s Love, Decency & Kindness

Qualifications

• Registered Nurse with minimum five (5) years clinical and/or community care experience.
• Current registration with AHPRA is required.
• Commitment to high‑quality, person‑centred care.
• Willingness to develop dementia care practice.

Responsibilities

• Coordinate resident assessments, care planning, reviews and evaluations
• Build trusted relationships with residents and families, monitoring wellbeing
• Explain care and funding options clearly and compassionately
• Connect residents with health and support services as required
• Collaborate with GPs, Nurse Practitioners, allied health and external providers
