Job summary

Infinite Aged Care Cornubia is seeking an experienced Clinical Care Coordinator & Nurse Mentor to join our Not-for-Profit team. Work a Monday–Friday roster with 2:00pm–10:00pm shifts, delivering high‑quality, person‑centred care and leading a clinical team across RNs, ENs and PCWs.

You will oversee care plans, coordinate resources, ensure regulatory compliance, and mentor staff to foster clinical excellence. A strong background in aged care and AHPRA registration is essential.

Qualifications

• Minimum 2–3 years experience as a Clinical Care Coordinator or similar in an aged care setting.
• Current unrestricted Registered Nurse Div 1 with AHPRA.
• Postgraduate qualifications in aged care would be advantageous.
• Experience with AN-ACC and accreditation documentation.

Responsibilities

• Provide strategic and operational leadership to the clinical care team to ensure high-quality, resident-centred care.
• Oversee development, implementation, and regular review of individualised care plans aligned with best practice.
• Monitor clinical activities and daily events to maintain service excellence.
• Mentor and support RNs, ENs, and PCWs to strengthen clinical capability and professional growth.
• Collaborate with multidisciplinary teams to coordinate holistic resident care.
• Facilitate effective communication processes such as shift handovers and team meetings.
• Provide oversight of clinical decision-making ensuring compliance with evidence-based practice.
• Lead onboarding, training, and ongoing development of care staff.
