Job summary

Lutheran Homes Group in Horsham, Victoria, is seeking a Registered Nurse for night duty to deliver person-centred care to residents and support the care team.

You will work with the team to plan and document care, communicate with families, and participate in quality improvement while upholding safety and dignity.

AHPRA RN registration is required, with at least 12 months' experience and ability to work independently and as part of a collaborative team.

Qualifications

• AHPRA Registration as Registered Nurse.
• Familiarity with Lee care is preferred; ability to learn new systems quickly is acceptable.
• Able to keep things running smoothly, even during busy times.
• Organised, motivated, and reliable, with the ability to work both independently and as part of a team.

Responsibilities

• Assisting the Care Coordinator with clinical documentation, care planning, and leadership of the care team.
• Leading and supporting new consumer admissions and care planning processes.
• Communicating effectively with families, allied health professionals, and other care partners.
• Fostering a welcoming, inclusive environment for residents and staff.
• Actively participating in quality improvement and risk management activities.
