Job summary

Whole Care Solutions in Caroline Springs, Melbourne VIC, is seeking a capable, organised and people-focused Service Delivery Manager to coordinate participant supports, workforce rostering and the day-to-day delivery of services across our supported living operations.

This full-time role is based from our Caroline Springs office and initially offered as a 6-month fixed-term contract with a pathway to permanent employment.

Qualifications

• Experience in disability, NDIS, SIL, community services.
• Strong organisational and communication skills.
• Ability to make operational decisions and coordinate multiple moving parts.

Responsibilities

• Coordinating day-to-day participant service delivery and operational follow-up.
• Managing staff rosters, workforce allocation, vacancies and continuity of support.
• Coordinating routine participant appointments and ensuring recommendations are followed through.
• Reviewing feedback and ground-level information from Team Leaders and Support Workers.
• Reviewing support plans, risk assessments, service documentation, reports and incident information to ensure they are accurate, current and translated into practical actions.
• Monitoring participant wellbeing, goals, risks and support-plan implementation.
• Coordinating responses to significant incidents, complaints and material participant changes.
• Planning house and team meetings in conjunction with Team Leaders.
