Job summary

Chorus is seeking a part-time Permanent Support Worker Multi Skilled to help seniors and NDIS customers remain in their homes across our WA communities. You will deliver tailored person-centered services and activities of your choosing, working with diverse individuals and families to support independence and wellbeing.

Ideal candidates hold a Certificate III in Individual Support (Aged Care or Disability) or an equivalent qualification, have strong communication skills, and can travel to

Qualifications

• Certificate III in Individual Support (Aged Care or Disability) or equivalent qualification/experience.
• Experience with seniors, mental health recovery, and disability.
• Strong communication with diverse customers, families, carers, and professionals.
• Enthusiastic and engaging with customers.
• Understanding of boundaries, privacy and confidentiality issues.
• Willing to provide domestic assistance services.
• Ability to work independently and as part of a team.
• Proactive approach to safety and well-being of customers.
• Current Driver's Licence with no restrictions.
• Reliable and roadworthy vehicle to travel to customers' homes and access the community.
• Comprehensive motor vehicle insurance and registration.
• NDIS Worker Screening Check (or willingness to apply).
• Current First Aid Certificate (12 months or less currency) or willingness.

Responsibilities

• Provide tailored person-centered services and activities to seniors and NDIs.
• Interact with customers, families, carers and professionals to support home-based living.
• Travel to clients' homes using own vehicle and ensure safety and privacy.
