Job summary

Super Rehab Pty Ltd is seeking a Community Physiotherapist to support older adults and NDIS participants in the Western Sydney area, delivering care in clients' homes with a mobile, client-focused model.

You will conduct comprehensive assessments, develop individualised treatment plans, and provide falls prevention, pain management, and functional support, with travel to clients' residences and a strong emphasis on work-life balance and professional growth.

Qualifications

Physiotherapy degree (essential)
Full AHPRA Registration (essential)
NDIS Worker Check (or willing to apply)
Current Driver's Licence + access to your own vehicle (essential)
Professional Indemnity & Public Liability Insurance (essential)
Some aged care and/or disability experience (desirable)

Responsibilities

Conduct comprehensive physiotherapy assessments focused on mobility, strength, balance, and functional independence.
Develop and implement individualized treatment plans, including exercise programs and mobility retraining.
Provide expert guidance on falls prevention, pain management, and manual handling strategies.
Support NDIS participants through functional assessments, capacity-building interventions, and reports for assistive equipment and home support needs.
