Job summary

Life Without Barriers in Fremantle is seeking a Casual Mental Health Support Worker to deliver psychosocial support to young people aged 17–22 experiencing homelessness or risk of homelessness. The role runs on a 24/7 roster with shifts including mornings, afternoons and occasional nights.

You will work to empower clients, develop support plans, and collaborate with community mental health services and carers to provide integrated care.

Qualifications

• Certificate IV in Mental Health (or equivalent experience)
• Experience working with people who have mental health issues
• Experience with homelessness or risk of homelessness
• Ability to engage with people from diverse cultural backgrounds, including Aboriginal and Torres Strait Islander peoples
• Ability to work with people with complex needs including psychiatric disability
• Current First Aid Certificate
• Current Driver Licence

Responsibilities

• Support consumers to develop their own support plans with Life Without Barriers and carers
• Collaborate with consumers and external services to provide integrated care
• Monitor progress and provide case notes to partner organisations
• Understand consumer expectations and health, family and cultural background
• Coordinate service delivery and refer to other agencies as needed

About the role

The residential program based in Fremantle provides community-based homeless youth services to young people between 17 and 22 years of age, who show signs and symptoms of mental illness while homeless or at risk of homelessness.

As a Mental Health Support Worker, you will provide psychosocial support which will focus on client determination, empowerment and hope.

This is a casual position which requires flexible and reliable staff who are able to commit to various shifts available across a 24/7 roster, including days (7:30am – 3:30pm), afternoon (2:30pm – 10:30pm) and occasional active night shifts (10pm – 8am).

Key Responsibilities

• Support consumers to develop their own support plans in partnership with Life Without Barriers, Community Mental Health services and carers
• Liaise and collaborate with consumers, their community and community support agencies such as mental health, housing and other services to ensure an integrated and timely service.
• Monitor and review consumers' progress through the service and against the case plan and provide up to date progress reports to the partner organisations.
• Seek to understand consumer's expectations.
• Sensitively ascertain information about the consumer's health, family and cultural background.
• Coordinate the delivery of services to consumers and work with the client to utilise other referral agencies and services.
• Observe and accurately record the client's behaviour, needs and interactions with others.
• Maintain accurate case notes, up to date case plans and files.
• Effectively plan and organise own work effort and regularly provide feedback to the manager on their work progress.

Skills & Experience

• Certificate IV in Mental Health (or equivalent experience)
• Experience working with people who have a mental health issue
• Previous experience working with people who are homeless or at risk of homelessness
• Demonstrated ability to work with people from Diverse Cultural Backgrounds including Aboriginal and Torres Strait Islander people
• Ability to positively engage and work with people who have complex needs including a psychiatric disability
• Current First Aid Certificate
• Current Driver Licence
