Job summary

Purple House in Alice Springs is seeking a full-time Case Manager to monitor client needs, coordinate services, and maintain documentation within a supportive Aboriginal community health setting.

You will work with Malpa Social Support teams, provide transport and personal care, advocate for patients, and engage with families and external stakeholders to ensure culturally safe, high-quality care in remote communities.

Qualifications

• Experience delivering Community, Aged and Disability Care Services.
• Demonstrated computer skills including Word, Excel, Outlook.

Responsibilities

• Walk beside and be a Malpa for dialysis patients and families.
• Deliver direct care services including transport, personal care, social and cultural activities.
• Advocate for patients and support holistic, culturally safe care.
• Collaborate with clinical teams to enable return to country trips and dialysis on country.
• Maintain documentation and participate in quality audits.
