NSW Health is seeking a Level 3 Physiotherapist to join the Westlakes Community Rehabilitation Team on a temporary part-time basis (18 hours per week) until June 2027. Westlakes Community Rehabilitation Team provides care to a diverse, interesting range of patients. Populations include subacute stroke, multi-morbidity, frailty and falls and neuro-degenerative illnesses.
This position in located in interdisciplinary team, our Physiotherapist works with our Team Leader, Speech Pathologist, Social Worker, Occupational Therapist, Enrolled Nurse, and Allied Health Assistant. Westlakes Community Rehabilitation Team has a reputation for excellent communication skills. Teamwork, patient centred outcome focused care.
The working days for this role are Wednesday - Friday
Our Physiotherapist takes the lead in managing our outpatient rehabilitation gym Monday to Friday mornings. Supported by our Team Leader, Allied Health Assistant, Enrolled Nurse, and other members of the interdisciplinary team. After preliminary individualised rehabilitation assessment appropriate patients attend a program of rehabilitation in our outpatient gym, their own home, community settings or via telehealth. Location of care is based on patient centred goals. Patients work on individualised programs in a group setting or on individualised programs at home, community, and in telehealth. Afternoon sessions can be completed 1:1 in our outpatient gym, home and community environments and via telehealth as relevant to patient goals.
Our Physiotherapists support patients to translate rehabilitation exercises into function within the community. This may include supporting patients to transition to community-based exercise programs. Our Physiotherapist collects and collates standardised and patient-centred outcome measures. This data informs quality improvement processes to further improve patient-centred care. In this role our Physiotherapist takes on several interdisciplinary roles and works effectively with the Allied Health Team. This may include interdisciplinary intake assessments, case management and coordination, complex report writing, team administration and discharge planning.The Physiotherapist will be supported by their Team Leader. Professional support will be provided by the Community Physiotherapy Head of discipline. The Physiotherapist will be supported to attend Community Physiotherapy meeting and professional development opportunities. Westlakes Community Rehabilitation has a well‐developed orientation system. Written administration guides, peer support and supervision will be available to orientate to this role.
Our Physiotherapist must enjoy team focused, patient-centred outcome‑based care. They will work well with others and enjoying learning and taking on interdisciplinary roles. The Physiotherapist will have a broad range of clinical rehabilitation skills. Be a problem‑based learner. Enjoy the challenge of working with diverse populations. Researching and learning new clinical skills. Within this team, the Physiotherapist will be provided with support to develop specialist skills in behaviour change, falls prevention, subacute rehabilitation and care of patients with advanced neurological disease.
