Job summary

Care and Connects Pty Ltd is seeking a compassionate Clinical Registered Nurse with leadership skills to join our dedicated team in Adelaide. You will oversee clinical care, medication compliance and risk mitigation while guiding a team to deliver high-quality, participant-centered care.

Responsibilities include conducting health assessments, developing and reviewing care plans, delivering training, and collaborating with participants, families and staff to achieve exceptional health outcomes.

Qualifications

Bachelor of Nursing and AHPRA registration.
Minimum of 3 years’ experience in aged care or disability services.
Valid First Aid/CPR, WWC, NDIS Worker Screening, Police Check and Australian Driver’s License.

Responsibilities

Conduct health assessments and develop individualised care plans.
Provide leadership to support workers, ensuring best practices in clinical care.
Deliver health-related education and training to participants and caregivers.
Develop, implement, and regularly review care plans with participants and families.
Monitor participant progress and compliance with NDIS standards.
Respond to clinical incidents and manage risk mitigation strategies.
Collaborate with multidisciplinary teams to enhance care quality.
