Job summary

City of Greater Geelong is seeking a dynamic Coordinator Community Care Programs to lead delivery of Food Services, Home Maintenance, Intake and Service Establishment, and community programs. You will work with the Care Services Lead to translate insights into practical improvements that enhance service quality for older people and carers.

Qualifications

• Relevant tertiary qualification in Community Services, Health, Human Services, Aged Care or related discipline, or extensive equivalent experience.
• Demonstrated experience leading complex operational programs within aged care or local government.
• Experience leading service improvement, program development and workforce planning.
• Experience managing service delivery, budgets and contractor relationships.
• Strong people leadership and change management skills.
• Ability to translate data into actionable improvements.

Responsibilities

• Lead design, implementation and review of program models that respond to community need, while maintaining compliance, quality and financial sustainability.
• Ensure services are delivered in accordance with the Aged Care Act, Aged Care Quality Standards, funding agreements, Council policies, reporting requirements and procedures.
• Analyze incidents, complaints, feedback, missed services and performance data to identify systemic issues and drive improvements.
• Lead, coach and develop direct reports to deliver safe, inclusive, customer-focused programs.
• Provide leadership through reform, service redesign and operational change for staff understanding of priorities.
• Develop, implement and review operational plans that support service sustainability and workforce capacity.
