Job summary

Opal HealthCare is opening Carlingford Greens Care Community in Sydney's north-west, and is seeking a Clinical Care Coordinator to help shape culture and clinical standards from day one. As an experienced Registered Nurse, you will lead clinical assessments, care planning and mentor nursing teams to deliver safe, high-quality, person-centred care.

This is a permanent, full-time role with flexible shifts across a 7-day roster.

Qualifications

• Current AHPRA registration as a Registered Nurse
• Minimum of 2 years’ experience as a Registered Nurse
• Strong clinical, care planning and AN-ACC experience
• Educator experience highly regarded

Responsibilities

• Lead clinical assessments, care planning and ongoing review of resident outcomes
• Coordinate resident care and support timely responses to changing clinical needs
• Provide clinical leadership, coaching and support to Registered Nurses, Enrolled Nurses and Care Support Team members
• Build trusted relationships with residents, families and healthcare professionals
• Support clinical governance, quality improvement and compliance with the Aged Care Quality Standards
• Contribute to a positive team culture focused on excellence, collaboration and person-centred care
