Jobstar, a standalone Residential Aged Care facility in Australia, seeks a Care Manager to lead our clinical and care teams with dignity, respect, and person-centred values.

You will deliver high-quality case management, assess, plan, and monitor residents’ needs, and collaborate with residents, families, and staff to achieve outcomes that enhance quality of life. This leadership role is part of our senior team and requires commitment to safe, inclusive care.

Qualifications

• Current practising Certificate as a Registered Nurse with AHPRA.
• Strong leadership and communication skills.
• Strong understanding of AN-ACC requirements.
• Strong understanding of the Aged Care Quality Standards.
• Extensive experience in delivering and supervising clinical care in an Aged Care environment.
• Strong customer service focus to meet residents' and families' expectations.

Responsibilities

• Provide clinical expertise, leadership, management, direction and guidance to clinical and care teams.
• Lead high-quality case management including assessment, planning and monitoring of residents' needs.
• Collaborate with residents and families to deliver person-centred care and positive outcomes.
