NSW Justice Health & Forensic Mental Health Network is seeking an Integrated Care Coordinator (HM1) for a Temporary Full Time role in the Sydney Metro/MRRC area. The role supports adult patients with chronic or complex health conditions, coordinating care across reception, custody, and community transitions.

Salary ranges from $91,326 to $121,497 per year plus superannuation. The position offers a supportive environment, opportunities for training and education, and a pathway to enhance patient

Qualifications

• Responsible for coordinating care for adult patients with chronic and/or complex health conditions.
• Provides support network to Network clinicians to assist with continuity of care, transfer of care into the community and post-release follow up.

Responsibilities

• Coordinate care across the patient journey from reception, through custody and into the community.
• Support clinicians to ensure continuity of care and post-release follow-up as required for custodial patients.
• Assist with case management and transfer of care into the community.
