Job summary

The Role

The Connections Program aims to improve continuity of care for patients who are in public correctional Centre with Drug and Alcohol problems, that are being released to the community. The Connections Program works within anassertive linkage model that aims to link patients with relevant health and welfare service providers pre and post release.
