Job summary

Amazing Care, Inc. is seeking a Licensed Physical Therapist to deliver in-home therapy to adult clients in Brighton, CO.

You will create and implement personalized treatment plans to improve mobility, strength, and safety, working directly with patients in their homes. Qualified candidates are Colorado-licensed PTs with home health or related experience, excellent communication, and proficiency with EMR documentation.

Qualifications

• Licensed Physical Therapist in Colorado.
• Experience in home health or a related field.
• Strong communication and interpersonal skills.
• Proficient in electronic medical record documentation.

Responsibilities

• Develop and implement individualized treatment plans.
• Conduct assessments using standardized tools and clinical judgment.
