Job summary

Whiddon is seeking a Community Coordinator for our Tamworth office to lead and develop social, family and community networks for older Australians in our care. This fixed-term, full-time role supports care plans, assessments and goals to help clients live safely and independently within the community.

You will work with a collaborative team, manage case coordination, and engage with Home Care Packages, Commonwealth Home Support Program, NDIS and ICare while promoting high standards of service

Qualifications

• Leadership and people management experience.
• Experience in community care coordination or be a Registered Nurse with AHPRA (not essential).
• Current Class C driver’s licence.
• NDIS Worker Check - NDISWC (or willing to obtain).

Responsibilities

• Working with the team to deliver high-quality services.
• Liaising with other service providers.
• Managing clients’ community care programs (Home Care Packages, CHSP, NDIS, ICare).
• Case management.
• Development of Day Programmes.
• Supporting staff to maintain high standards of care.
