Job summary

Life Without Barriers in Bunbury is seeking a dedicated Hub Home Provider to support Satellite Families and children in care. You will create a safe, welcoming Hub Home, coordinate meetings and connect families with education, mental health and community services.

The role emphasizes building trusted relationships with carers, managing placement challenges, maintaining records, and working closely with staff and community services to promote positive outcomes for children and young people.

Qualifications

• Experience as a foster or kinship carer (5+ years preferred), or relevant experience in Child Protection or Out of Home Care.
• Ability to meet the requirements to obtain and maintain authorisation or registration as a foster carer under the relevant jurisdiction.
• Strong relationship building and communication skills, with the ability to work positively with carers, children, young people and other stakeholders.
• Ability to support children and young people and respond appropriately to placement challenges and changing needs.
• Current Provide First Aid (HLTAID0011) Certificate.
• Current Driver’s Licence.

Responsibilities

• Coordinate and host monthly Constellation meetings, quarterly social outings and activities for carers, children and young people.
• Provide regular support to Satellite Families and children and young people, including assistance with placement challenges and crisis situations.
• Connect families with education, mental health and community services to support positive outcomes.
• Host case planning, care team and family meetings at the Hub Home where appropriate.
• Build strong relationships with carers, children and young people and provide a welcoming and supportive Hub Home environment.
• Maintain accurate records and work collaboratively with carers, Life Without Barriers staff and community services.
