Are you an experienced Registered Nurse who enjoys clinical variety, independent decision-making and making a real difference to patients? Join our Hospital in the Home (HiTH) team and deliver high-quality, hospital-level care to people in the comfort of their own homes. You'll help patients receive the right care in the right setting, supporting recovery while avoiding or reducing the need for an extended hospital stay.

Enjoy the flexibility and autonomy to plan your day. You’ll be allocated a patient workload and can use your clinical judgement to prioritise visits, plan an efficient route and organise your time around patients' needs, while remaining connected to the wider HiTH team.

You’ll work closely with HiTH medical officers, GPs, acute hospital teams and community providers to coordinate safe, individualised care and support smooth transitions between hospital and home. You'll also have the opportunity to build your community nursing skills in a supportive team environment.

Enjoy autonomy and variety in your day
  • Assess and triage referrals from acute hospitals, determining patients' suitability for Hospital in the Home services.
  • Complete comprehensive nursing assessments and clinical observations, monitor progress and respond confidently to changing clinical needs.
  • Coordinate admissions and discharges, supporting safe transitions and continuity of care.
  • Provide hospital-level care in patients' homes, including medication administration, wound care and pathology collection as required.
  • Support chronic disease management and provide clear, practical education to patients and their families about their condition, treatment and care plan.
  • Liaise with HiTH medical officers, GPs and the multidisciplinary team to coordinate individualised care and achieve positive patient outcomes.
Bring your clinical confidence to the community
  • Strong clinical assessment and critical-thinking skills, with the confidence to make sound decisions and deliver safe, evidence-based nursing care.
  • Experience in Hospital in the Home, Emergency, Critical Care or a similar acute setting is highly regarded.
  • Experience supporting chronic disease management and educating patients to confidently manage their health and care at home.
  • Excellent communication skills and a patient-centred approach, with the ability to advocate for patients and work effectively with families and healthcare providers.
  • Confidence working independently and prioritising care when you are the sole clinician in a patient’s home, while knowing when to seek support.
  • Demonstrated computer literacy, including email and electronic patient record systems.
  • A commitment to ongoing professional development, reflective practice and continuous improvement.
  • Strong organisation and time-management skills, with the ability to plan your workload, prioritise visits and manage competing clinical needs.
Essential Criteria
  • Current registration as a Registered Nurse with AHPRA.
  • A valid, unrestricted driver's licence and willingness to travel to patients' homes to deliver hospital-level care in the community.

  • The opportunity to broaden your clinical assessment, chronic disease management and community nursing skills.
  • Make a meaningful difference by helping patients receive hospital-level care at home and supporting positive recovery outcomes.
