Job summary

UPA Aged Care Solutions is seeking a Home Care Worker to support clients in the Northen Rivers Region, based in Grafton. You will provide person-centred care, assist with daily living tasks, and help clients remain independant at home.

Qualifications

• Certificate III in Individual Support or equivalent (Aged Care/Community Services).
• Current Australian Driver’s Licence and willingness to travel.
• Registered and insured vehicle.
• Compassion, empathy and passion for helping others.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Confidence in using technology.

Responsibilities

We currently an opportunity for Home Care Worker to join our Northen Rivers Region team based in Grafton.

Your Responsibilities:

• Deliver individualised care using a person-centred approach as per client care plans
• Assist with personal care, medications, transport, and respite services
• Provide everyday living support such as domestic assistance, gardening, shopping, and meal preparation
• Complete accurate and timely reporting of client services
• Identify and report client risks via our incident management system

What You’ll Need:

• Certificate III in Individual Support, Aged Care, or Community Services (highly desirable)
• Current Australian Driver’s Licence and willingness to travel
• Registered and insured vehicle
• Compassion, empathy, and a passion for helping others
