Job summary

Empower Healthcare in NSW invites a qualified Physiotherapist to join our Home Care team, delivering musculoskeletal, neurological and cardiorespiratory care to participants in their own homes across Sutherland and St George.

You will conduct comprehensive assessments, develop tailored treatment plans, collaborate with other professionals, educate families, and ensure clear communication with service providers.

Qualifications

• Bachelor of Physiotherapy (or equivalent) is required.
• Registration with AHPRA.
• Police Check Clearance.
• Excellent verbal and written communication skills.
• Strong time-management skills and attention to detail.

Responsibilities

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home via Support at Home/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan.
• Educate participants and their families to optimise their ability to live healthy lives within the community.
• Liaise with service providers to ensure effective communication and maintain records and patient reports.

The Position

Our innovative service model focuses on delivering the best clinical outcomes for participants. As a Physiotherapist with Empower, you will have the opportunity to work with a variety of Musculoskeletal Neurological and Cardiorespiratory patients.Working in a supportive team environment, you will be responsible for the delivery of Empower Healthcare's comprehensive services across Support at Home and NDIS participants in Sutherland and St George. You will:

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan
• Educate participants and their families to optimise their ability to live healthy lives within the community
• Liaise with service providers to ensure effective communicationWork autonomously to prioritize and complete tasks, maintain records and provide patient reports

You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.
