Job summary

HammondCare is seeking a Care Partner to join the Home Care team in the Lismore area, supporting clients across Kyogle and surrounding communities. This full-time role offers guaranteed hours and a hybrid work model, with duties including developing tailored care plans and ongoing client assessments in consultation with stakeholders.

You will coordinate care delivery, manage client-related KPIs, ensure plans are current and aligned with funding and service expectations, and support staff with

Qualifications

Certificate IV in Aged Care and at least 2 years in the industry.
Knowledge of Support at Home, CHSP and Aged Care Quality Standards.
Experience in a multi-site environment and interdisciplinary teams.
Current driver’s license and willingness to travel between clients/sites.
Strong administrative and computer skills.

Responsibilities

Assist in delivering services tailored to client needs within budget and service expectations.
Manage existing clients and client-related KPIs.
Ensure care plans are up to date and aligned with funding and service expectations.
Provide day-to-day delivery of services and ensure staff understand individual client needs and preferences.
