Job summary

Trafalgar Community Care Ltd is seeking a Home Care and Support Assistant to provide person-centred care in clients' homes across NSW. You will promote independence, dignity and wellbeing while delivering practical support.

The role covers personal care, mobility, meals, medication prompting, companionship, and light household tasks. You will follow care plans, maintain records, and report changes.

Qualifications

• Previous care experience is desirable but not essential; full training provided.
• Good communication and interpersonal skills are essential.
• Willingness to undertake training and development.

Responsibilities

• Assist with personal care including washing, dressing, grooming, Toileting.
• Support mobility including safe transfers and use of mobility aids.
• Help with meals, nutrition and hydration.
• Prompt or administer medication per care plans.
• Provide companionship and emotional support.
• Assist with light household duties and errands.
• Support attendance at appointments and social activities.
• Maintain accurate care records and report changes.
• Follow care plans and risk assessments for safe delivery.
• Adhere to safeguarding procedures and report concerns.

To provide high-quality, person-centred care and support to individuals in their own homes, promoting independence, dignity, and overall wellbeing.

Key Responsibilities:

• Assist clients with personal care, including washing, dressing, grooming, and toileting
• Support with mobility, including safe transfers and use of mobility aids
• Help with meal preparation, feeding, and maintaining adequate nutrition and hydration
• Administer or prompt medication in line with care plans and company policy
• Provide companionship and emotional support
• Assist with light household duties such as cleaning, laundry, and shopping
• Support clients with attending appointments or social activities where required
• Maintain accurate records of care provided and report any changes in a client's condition
• Follow care plans and risk assessments to ensure safe and effective care delivery
• Adhere to safeguarding procedures and report any concerns appropriately

Skills and Experience:

• Previous experience in care is desirable but not essential (full training provided)
• Good communication and interpersonal skills
• Compassionate, patient, and reliable approach
• Ability to work independently and as part of a team
• Basic understanding of health and safety and safeguarding principles

Qualifications:

• NVQ Level 2 or 3 in Health and Social Care (desirable but not essential)
• Willingness to undertake training and development

Other Requirements:

• Flexible availability, including evenings and weekends
• Full UK driving licence and access to a vehicle (desirable, depending on role)
• Enhanced DBS check required

• Work is carried out in clients' homes
