Uniting Vic.Tas seeks a Placement Support Worker to collaborate with children, young people, families, carers and stakeholders to ensure outcomes in case plans are achieved. The role includes ongoing training and supervision for caregivers and supporting environments within caregiving households.
The program delivers high quality case management for vulnerable children aged 0-18 years, placed with volunteer caregivers through emergency, respite, short-term or long-term placements.
As a Placement Support Worker, you will work collaboratively with children, young people, their families, carers and other stakeholders to ensure desired outcomes are achieved in the context of the child or young person’s needs and case plan. The role will also provide ongoing training and supervision to caregivers, and actively support the development of positive relationships and environments within caregiving households.
The Home Based Care Program provides high quality case management support to vulnerable children and young people aged 0-18 years who are unable to live with their own families and are placed in the care of volunteer caregivers through emergency, respite, short term or long term placements.
