Job summary

Geelong Careers seeks a Care Manager to deliver high-quality management of home-based aged care services in Geelong. You will coordinate participant-centred care, manage service delivery and budgets, and support participant choice and independence.

This permanent full-time role involves leading care planning, monitoring changing needs, and contributing to program quality and improvement, while promoting wellbeing and reablement.

Qualifications

• Formal health qualification required for care management.
• Experience delivering home-based care is essential.
• Clear written and verbal communication skills and computer literacy.

Responsibilities

• Manage a caseload of home-based participants with individualised care planning.
• Monitor changing needs and respond promptly to ensure supports remain appropriate.
• Ensure care plans align with participant goals and promote independence and wellbeing.
• Develop, manage and review participant budgets on a quarterly basis.
• Monitor package expenditure and contribute to program budget management to utilise funds.
