Ozcare seeks an Allied Health Assistant to join the Rockhampton Aged Care at Home team. You will support clients to achieve goals, maintain independence, and enhance wellbeing in their own homes and communities.

You will implement therapy programs, support rehabilitation and wellbeing goals, monitor progress, and document outcomes while collaborating with a multidisciplinary team and upholding quality, safety and compliance standards.

Competenze

• Certificate IV in Allied Health Assistance or equivalent qualification.
• Experience supporting aged care clients and/or people living with disability.
• Current Police Screening Certificate (valid within the past 3 years) or willingness to obtain.
• Current Queensland Driver Licence.
• Current First Aid & CPR Certificate.
• Vaccination against MMR, Varicella and Pertussis or willingness to become vaccinated.

Mansioni

• Assist with delivery of therapeutic programs under Allied Health Professionals.
• Support clients in their homes to rehabilitation, reablement and wellbeing goals.
• Monitor, document and report client progress and responses to therapy interventions.
• Assist with client goal reviews, assessments and evaluations as directed.
• Prepare, maintain and transport therapy equipment and aids.
• Record accurate client documentation and progress notes.
• Support administrative tasks including scheduling and records management.
• Participate in multidisciplinary meetings and collaborate with stakeholders.
• Maintain client confidentiality and comply with policies and standards.
