South Gippsland Hospital (SGH) invites applications for a Health Care Worker to join our Support at Home program in Victoria. The role focuses on helping clients live safely at home, in line with their goals and preferences.

To be successful, candidates will hold a Certificate III or IV in Aged Care or Community Services, have experience with frail aged people or disabilities, and demonstrate teamwork, communication and time management.

Qualifications

• Certificate III or IV in Aged Care, Community Services or equivalent.
• Demonstrated commitment to improving quality of life for clients.
• Well-developed interpersonal, communication and organisational skills.

South Gippsland Hospital (SGH) is a public rural health service providing integrated acute hospital and community care services. Care is delivered through our Community Health Centre, Banksia Centre, and in clients' homes across the region. In partnership with the doctors of Foster Medical Centre , SGH supports the health and wellbeing of the Foster community and surrounding areas, including Wilsons Promontory.

South Gippsland Hospital is seeking a motivated Health Care Worker to join our team. The role is responsible for assisting clients within our Support at Home program, helping them remain living safely at home in accordance with their individual goals and preferences.

To be successful in this role, you will have:

• Certificate III or IV in Aged Care, Community Services or equivalent
• Appropriate qualifications with a commitment to further education as required.
• Demonstrated experience working with frail aged people or people with disabilities and their carers.
• Demonstrated commitment to enhancing the quality of life of all clients and consumers.
• Well-developed interpersonal, communication and organisational skills and is able to take direction and have excellent time manage skills.
• Demonstrated ability to work within a team or independently as required
• Ability to identify and seek appropriate support and guidance when required.
• Be solution focused, positive and have a capacity to respond effectively to challenges.
• Professional presentation, as appropriate for the occasion.
• Demonstrated ability to report accurately to supervisors as required.
