Job summary

Continuity Care is building a coordinated care ecosystem in Australia, joining disability, aged care and primary health around the person. We are seeking a Business Development Manager to lead growth across NDIS and community aged care, forging partnerships, referral networks and government relationships to reduce fragmentation and improve outcomes.

You'll own four priorities: client growth, awareness with regulators, platform growth, and integrated care revenue, while building a high-performing

Qualifications

• 5+ years' progressive business development or growth experience in NDIS/aged care
• Proven ability to build growth programs and infrastructure
• Strong sales target achievement with pipeline management
• Experience engaging multi-stakeholder groups including government
• Knowledge of NDIS funding streams and Home Care Package/Support at Home
• Excellent communication across B2B, government and client conversations
• Proficiency with CRM, marketing automation and analytics tools, plus AI tools
• Experience building and leading a team with budgeting and ROI
• Self-starter capable of juggling a broad territory autonomously

Responsibilities

• Build and own referral pipeline across NDIS supports and aged care
• Establish high-yield relationships with coordinators, plan managers, allied health professionals
• Design and run multi-channel demand programs and campaigns
• Represent Continuity Care at disability expos and sector networks
• Build engagement with Queensland Government departments and health networks
• Develop the platform's value proposition and recruit Associate Providers
• Lead rollout of the Integrated Model of Care with hospitals and GPs
• Stand up growth systems: CRM, marketing automation, AI workflows
• Build and mentor a high-performing BD & Engagement team
• Report weekly pipeline and monthly growth results to the executive
