Responsibilities

• Provide patient-centred nursing care within the geriatric rehabilitation unit.
• Collaborate with nursing, medical and multidisciplinary teams to achieve safe, high-quality outcomes.
• Support discharge planning and patient-family engagement throughout rehabilitation.
