Job summary

Connect2Care seeks a therapist to join a multidisciplinary team delivering evidence-based services via telehealth, community visits and centre-based care across Australia. The role is mobile with options to work from home or hubs when not meeting clients.

The New Graduate Program supports recent graduates over 12 months, with induction, supervision, core training and mentoring to launch your career in allied health.

Qualifications

• Tertiary qualification in related field.
• Professional registration (e.g. AHPRA/SPA).
• Working with Children Check (WWCC).
• NDIS Worker Screening Check.
• Police Check & International Police Check (if applicable).
• Driver's license & access to a reliable vehicle.
• Car insurance.
• Professional Indemnity & Public Liability insurance.

Responsibilities

• Assess and treat clients across telehealth, community visits and centre-based services.
• Support NDIS participants to maximise function, capacity and social engagement.
• Assist with assistive technology assessment and applications as relevant.
• Maintain timely documentation and report writing.
• Manage a varied caseload and meet professional expectations (billables & client management).
• Attend meetings, supervision and PD sessions as required.
• Build relationships with clinicians, clients and referral sources.
• Operate across leading software platforms and online tools.

The Role

As a therapist at Connect2Care, you will be joining a multidisciplinary team. You will provide evidence-based services to people through multiple service delivery models, including telehealth, community visits and centre-based services. Referrals may come through the NDIS, Medicare or privately. It is a mobile role, with the option to work from home or our hubs when not seeing clients. The role will involve the following:

• Assessing and providing interventions for children & adults living with a range of disabilities
• Working within the NDIS to maximise clients' level of function, capacity & social engagement
• Assisting clients with assistive technology assessment, recommendations and applications (in relevant fields)
• Timely documentation & report writing
• Managing a varied caseload
• Meeting company professional expectations (e.g. meeting billables & effective client management)
• Attending meetings, supervision & personal development (as required)
• Building relationships with key stakeholders, including therapists, clients, & referral sources
• Working across leading software platforms & online tools

The New Grad Program

Connect2Care has developed and specifically tailored a New Graduate Program to make the transition smoother for new graduates entering the workplace. The program fully supports recent graduates, enabling them to acquire the skills needed to launch them into a successful career. Spanning over 12 months and starting from the moment you step through our doors.

• Relevant tertiary qualifications in related field
• Discipline-specific professional registration (e.g. AHPRA/SPA)
• Working with Children Check (WWCC or Blue Card)

• Police Check & International Police Check (only applicable to individuals who have lived overseas)
• Drivers licence & access to a reliable vehicle
• Car insurance
