Barossa Hills Fleurieu Local Health Network is seeking a Program Manager GEM Service for Strathalbyn & District Health Service. Qualifications

• Multi-classed position suitable for Registered Nurse Level 3 or Allied Health Professional Level 3.
• Appointment commensurate with applicant's experience, demonstrated competence and capacity to perform with increasing independence, clinical judgement and complexity of practice.

Responsibilities

• Provide clinical leadership and coordination for the GEM Service, ensuring safe, effective and high-quality care for older patients throughout rehabilitation.
• Lead and support multidisciplinary teams, promoting collaboration, continuity of care, workforce development and a culture of excellence.
• Drive service improvement initiatives through quality, safety, risk management, data analysis and evidence-based practice implementation.
• Manage operational and program responsibilities including resource management, workforce planning, patient flow and service coordination.
• Build strong partnerships with internal and external stakeholders while contributing to strategic projects, education programs and innovative models of care.
