Job summary

Barossa Hills Fleurieu Local Health Network is seeking a Part-time Program Manager GEM for Strathalbyn & District Health Service. The role combines geriatric rehabilitation leadership with program development and multidisciplinary governance to improve patient outcomes in older adults.

You will provide clinical leadership, oversee GEM program growth, support education and workforce development, and collaborate with regional health services to drive service improvement in a supportive regional

Qualifications

Suitable for RN Level 3 or Allied Health Professional Level 3.
Appointment commensurate with experience, demonstrated competence, and capacity for independence.
Role requires clinical judgement and increasing complexity of practice.
Ability to lead and coordinate the GEM service.

Responsibilities

Provide clinical leadership and coordination for the GEM Service, ensuring safe, effective, and high-quality care for older patients throughout rehabilitation.
Lead and support multidisciplinary teams, promoting collaboration, continuity of care, workforce development, and a culture of excellence.
Drive service improvement initiatives through quality, safety, risk management, data analysis, and implementation of evidence-based practice.
Manage operational and program responsibilities including resource management, workforce planning, patient flow, and service coordination.
Build strong partnerships with internal and external stakeholders while contributing to strategic projects, education programs, and innovative models of care that improve patient outcomes.
