Job summary

Synergy Medical Practice is seeking an experienced Practice Nurse for a part-time role in St Leonards, NSW. The position offers approximately 10 hours per week with flexibility to split across two days and some tasks possible from home.

The successful candidate will join a supportive team delivering high-quality, patient-centred care, with duties spanning chronic disease management, immunisations, health promotion, and quality improvement activities.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Strong organisational and communication skills.
• Experience with chronic disease management and preventative health programs.
• Ability to work independently and within a small team.

Responsibilities

• Chronic disease management and care plan coordination.
• Health assessments and recalls.
• Immunisations and vaccine management.
• Patient education and health promotion.
• Assisting with quality improvement activities.
• Clinical triage and patient follow-up.
• Management of recalls and reminders.
• Support with accreditation and compliance activities.
• General nursing support to GPs and clinical team.
