Qualifications

• Experience with general practice care is preferred

Responsibilities

• Deliver immunisations and manage vaccine cold chain
• Support chronic disease management and care planning
• Perform ECGs, spirometry, wound care and health assessments
• Monitor blood glucose and assist with minor procedures
• Provide patient triage and telephone support
• Promote health and preventative care to patients
