Job summary

Physio To You is seeking a part-time Community Occupational Therapist to join our Melbourne team and deliver Support at Home services. You will assess clients at home, recommend equipment and home modifications, and support independent living with a client-centered approach.

You will work a 3-day-per-week schedule, with flexible weekdays and no weekend work, including kilometres reimbursement and a PD allowance.

Qualifications

• Current registration with AHPRA.
• Experience working with older adults in community or home settings.
• Strong clinical assessment, reasoning and report-writing skills.

Responsibilities

• Conduct home-based occupational therapy assessments under the Support at Home program.
• Assess daily living activities, mobility and home safety.
• Recommend assistive equipment and home modifications to improve safety and independence.
• Prepare comprehensive assessment reports and recommendations for clients and carers.
• Provide practical education to clients, families and carers for independent living.
• Collaborate with multidisciplinary team to ensure coordinated care.
• Maintain accurate clinical documentation and professional standards.
