Job summary

Amplar Health is seeking Enrolled Nurses for the Transition Care Service in Adelaide CBD or Walkerville. You will join a multidisciplinary team, supported by senior staff and a Nurse Educator, to help patients recover and build confidence in their care journey.

Permanent opportunities are available from 0.6 FTE to full-time. A patient-focused approach, wound care experience and vaccination evidence are valued as you work across a seven-day roster including morning, afternoon and night shifts.

Qualifications

• 1+ years’ experience as an Enrolled Nurse.
• Experience in wound care.
• Current CPR certification.
• A patient-focused approach and strong attention to detail.
• SA Working with Children Check and National Police Clearance, or willingness and eligibility to obtain these
• Ability to provide evidence of required vaccinations if successful

Responsibilities

• Deliver compassionate, person-centred nursing care
• Monitor patient progress and recognise and report clinical changes
• Contribute to assessments and individualised care planning
• Administer medications within your scope of practice
• Support wound care and activities of daily living
• Work alongside RNs and the broader multidisciplinary team
• Maintain accurate clinical documentation

Amplar Health Home Hospital’s Transition Care Service supports patients through an important stage of their healthcare journey, providing care as they recover, build confidence and prepare forwhatever comes next.

We’re looking for Enrolled Nurses who want to build their skills and confidence in a supportive, multidisciplinary environment at either the Adelaide CBD or Walkerville locations.

With experienced senior staff and a Nurse Educator by your side, you’ll have the support to settle in, strengthen your clinical skills and continue growing.

The role

No two patient journeys are the same. You’ll:
