Sydney Local Health District’s Community Health Centres have a proud history of delivering community care, including prevention, early intervention, assessment, treatment, health maintenance, and continuing care services aimed at improving community wellbeing. This role will be based across SLHD.
The Aboriginal Health Worker (AHW) will be part of a collaborative team providing culturally appropriate support and care in partnership with Chronic Care Coordinators to individuals with chronic diseases. The role involves establishing links with Aboriginal communities, identifying health barriers, and supporting access to health services. For more details, view the Position Description.
