Job summary

St Vincent's Private Hospital Toowoomba is seeking a dedicated Discharge Coordinator to join our team. You will support quality patient care and coordinate safe, timely discharges, liaising with medical staff and community services to ensure seamless transitions from hospital to home or next level of care.

The role requires current AHPRA registration as a Nurse and experience in discharge planning, with a commitment to compassionate, patient-centered care.

Qualifications

• Registered with AHPRA as a Registered Nurse.
• Experience in community and/or facility discharge processes preferred.
• Strong communication and collaborative skills for liaising with medical and nursing staff.

About the Opportunity

This role will be pivotal in delivering quality services to our patients and visitors in a caring, friendly atmosphere.

St Vincent's Private Hospital Toowoomba is the largest acute private hospital on the Darling Downs. We are a 197-bed facility with highly experienced and skilled staff that work as a team to deliver the highest standard of healthcare to all our patients. Things that work for us

• Registered with AHPRA (Australian Health Practitioner Regulation Agency) as a Registered Nurse
• Previous experience in community and/or facility (or similar role)
• Ability to work on a permanent full-time basis (38 hours per week)
• Effective communication skills at all levels enabling you to achieve best practice
• To provide a consultative and liaison focused service aimed at supporting medical and nursing staff in the assessment, care, management and successful discharge of patients with necessary family and community organisations to ensure the smooth and safe transition out of hospital.
• To ensure effective and timely admission/discharge processes are in place and to case manage patients with an extended length of stay
• You will understand the importance of providing quality care, and you feel rewarded by creating a genuine impact
• Well-developed interpersonal skills that provide the support and nurture for our patients and organisation's needs.
