Job summary

Empower Listen Flourish Pty Ltd is seeking a Disability Support Worker to provide individualized in-home and community support under NDIS guidelines. You will deliver compassionate, person-centered care, including intimate personal care and medication support where trained, within a trauma-informed framework.

Role involves domestic task assistance, health monitoring, and adherence to safety and documentation standards, with shift patterns including long, short, and sleepovers.

Qualifications

• Certificate III or IV in Individual Support (or equivalent) – desirable.
• NDIS Worker Screening Clearance.
• Working With Children Check (where applicable).
• First Aid and CPR.
• Driver's Licence and reliable insured vehicle.
• Manual Handling training.
• Medication competency (or willingness to obtain) – training will be provided.
• Completion of NDIS Worker Orientation Module and Infection Prevention & Control training.

Responsibilities

• Assist participants with domestic tasks including meal preparation, cleaning, laundry and maintaining a safe and hygienic living environment, while encouraging active participation to build daily living skills.
• Provide person-centered personal care in accordance with individual support plans, including assistance with bathing, dressing, grooming, toileting, ileostomy mobility and medication administration (where trained and delegated).
• Implement Behaviour Support Plans as written and utilise de-escalation strategies where required.
• Monitor participant health, behaviour and presentation, promptly reporting changes, risks or safeguarding concerns to management.
• Complete accurate, objective shift notes and incident reports within required timeframes.
• Uphold the NDIS Code of Conduct and comply with Empower Listen Flourish Pty Ltd policies and procedures at all times.
• Understand and implement individualised support plans, care plans and Behaviour Support Plans in accordance with documented strategies and organisational procedures.
• Maintain strict professional boundaries in accordance with the NDIS Code of Conduct.
