Job summary

Creating Links is a not-for-profit multicultural community service provider in South-West Sydney, dedicated to empowering people with disability to live independently. We are seeking compassionate Disability Support Workers to deliver high-quality, person-centred supports, built on respect, inclusion and cultural safety.

You will assist with daily living activities, accompany participants to community events, maintain clear records and collaborate with Coordinators to achieve NDIS goals.

Qualifications

• Certificate III in Individual Support (Disability) or related qualification.
• Experience supporting people with disability and/or families in direct support.
• Commitment to person-centred practice, inclusion and rights of people with disability.
• Strong interpersonal, verbal and written communication skills.
• Flexible to work evenings, weekends and public holidays.
• Current First Aid and CPR or willingness to obtain within 3 months.
• Knowledge of NDIS Practice Standards and WHS responsibilities.
• Current Working with Children Check and National Police Check.
• Class C Driver Licence and access to a vehicle for work-related travel.

Responsibilities

• Deliver high-quality, person-centred supports that promote independence, choice and inclusion.
• Support participants with daily living activities, including personal care, meals and medication assistance.
• Assist participants to access their community, build social connections and participate in recreational, educational and employment opportunities.
• Work with participants to achieve their individual NDIS goals and maximise independence.
• Build positive, respectful relationships with participants, families, carers and the wider support network.
• Maintain accurate case notes, progress records and documentation in line with NDIS Practice Standards and organisational requirements.
• Identify and respond to changes in participant wellbeing and report concerns promptly.
• Uphold the rights, dignity, privacy and cultural needs of every participant.
• Maintain a safe working environment and comply with legislation including NDIS standards, Child Safe Standards and mandatory reporting obligations.

In this role, you will

• Support participants with daily living activities, including personal care, meal preparation, household tasks and medication assistance where required.
• Assist participants to access their community, build social connections and participate in recreational, educational and employment opportunities.
• Work alongside participants to achieve their individual NDIS goals and maximise their independence.
• Build positive, respectful relationships with participants, families, carers and the wider support network.
• Maintain accurate case notes, progress records and documentation in line with NDIS Practice Standards and organisational requirements.
• Identify and respond appropriately to changes in participant wellbeing, reporting concerns promptly.
• Uphold the rights, dignity, privacy and cultural needs of every participant at all times.
• Maintain a safe working environment and comply with all relevant legislation, including NDIS Practice Standards, Child Safe Standards and mandatory reporting obligations.
• Work collaboratively with Coordinators and the wider Creating Links team to deliver outstanding outcomes for participants.

What we're looking for

We're seeking people who are compassionate, reliable and committed to supporting people with disability to live the life they choose. You'll be someone who genuinely enjoys building relationships, works well independently and as part of a team, and is committed to providing high-quality, person-centred support.

To be successful in this role, you'll have

• A minimum Certificate III in Individual Support (Disability), Community Services or a related qualification.
• Experience supporting people with disability and/or their families in a direct support role.
• A genuine commitment to person-centred practice, inclusion, choice and the rights of people with disability.
• Strong interpersonal, verbal and written communication skills.
• The ability to build positive relationships with participants, families and colleagues.
• An understanding of person-centred support, capacity building and promoting independence.
• The flexibility to work a variety of shifts, including evenings, weekends and public holidays as required.
• A current First Aid and CPR Certificate, or the willingness to obtain one within three months of commencing.
• Knowledge of the NDIS Practice Standards, Disability Inclusion Act 2014, child protection requirements and mandatory reporting obligations.
• An understanding of Work Health and Safety (WHS) responsibilities and safe work practices.
• A current unrestricted Class C Driver Licence and confidence driving company vehicles, including a 12-seat van.
• A reliable, comprehensively insured vehicle available for work-related travel.
• A current Working with Children Check.
• A current National Police Check (or willingness to obtain one before commencing).
• Competent computer skills, including the ability to use electronic systems for documentation and communication.

Desirable Criteria:

• An understanding and appreciation of the issues affecting people with a disability, the ageing population, and their families.
• Computer Literacy
• Fluency in a second relevant community language as well as English.
