Job summary

Healthscope Independence Services (HIS) is seeking a dedicated Disability Support Worker to provide high-quality care within our Supported Independent Living services in Victoria. You will support clients with daily living, personal care, and community activities, helping them pursue independence.

Ideal candidates hold a Certificate IV in Disability or related field, have at least 1 year of disability or health care experience, and demonstrate strong communication, organisation, and teamwork

Qualifications

• Certificate IV in Disability, Community Services, or related qualification.
• Minimum of 1 year of experience in disability or healthcare services.
• Flexibility to work across a range of shift types.

Responsibilities

• Provide personal care support in line with individual support plans.
• Assist participants with daily living activities in meal preparation and feeding.
• Assist with household tasks such as cleaning, laundry, etc.
• Accompany and support clients in community-based activities, such as social outings, appointments, and recreational pursuits.
• Ensure all required documentation is thorough and up to date.
• Experience with behaviours of concern
• Adhere to organisational policies, including medication administration, WHS, client rights, privacy, and confidentiality.
• Work within a team environment.

Job description

At the Healthscope Independence Services (HIS), we are seeking a passionate and experienced residential/support worker to provide high-quality to our clients within our Supported Independent Living (SIL) services operating within the NDIS.

• Classification: Level 2 - (Pay point will be determined based on experience and qualifications).

About the role

As a Disability Support Worker, you will be responsible for providing high-quality care and assistance to our clients, helping them achieve their goals and live independently. You will work closely with our clients, supporting them with daily living activities, personal care, and community engagement.

A typical day may include

• Provide personal care support in line with individual support plans
• Assist participants with daily living activities in meal preparation and feeding.

• Ensure all required documentation (including daily notes, communication and progress reports) is thorough and up to date.
• Experience with behaviours of concern
• Adhere to all organisational policies, including medication administration, WHS, client rights, privacy, and confidentiality.
• Work within a team environment.

To be considered for this role you will have

• Certificate IV in Disability, Community Services, or related qualification.
• Minimum of 1 year of experience in disability or healthcare services is required.
• Strong communication, written, interpersonal, and organisational skills.
• Flexibility to work across a range of shift types.
