Job summary

Guiding Grace Disability Services in Wonthaggi, VIC, is seeking committed disability support workers to join our growing team. You will deliver direct care aligned with each participant's plan and contribute to their daily living goals. Qualifications

• Deliver outstanding service and assistance to participants in accordance with their care plan and as directed.
• Support participants to access activities and outings in the community.
• Provide transport to participants as required or requested.
• Cook nutritious meals while following food safety guidelines for participants.
• Ensure completion of appropriate documentation and record in client notes.

Responsibilities

• Deliver outstanding service and assistance aligning with each participant's care plan.
• Collaborate with clients, family/careers, case managers and other professionals to support the client's individual goals.
• Maintain documentation and client notes, and communication book as required.
• Report any quality and safety incident or hazards to management.
