PURPOSE
The Disability Support Worker provides and delivers individual and personalised care and support to participants, in line with their needs and choices, so that they may achieve their goals as stated in their NDIS plans.The Disability Support Worker's responsibilities may include personal care, preparing meals and other health and wellness related activities, house cleaning, transportation, and communicating relevant information to support. The Disability Support Worker will demonstrate a commitment to safe and effective practices, providing services in a caring and respectful manner in accordance with Sydwest and Riverlink's policies and procedures.
