Job summary

Far West NSW Local Health District – Broken Hill NSW is seeking a Senior Allied Health Clinician to deliver advanced assessment and care coordination for people with disability and complex health needs across Far West LHD.

You will support the Hospital Social Work team, coordinate care across NSW Health, disability providers and community agencies, and drive service development while building strong stakeholder relationships for better patient outcomes.

This position qualifies for an annual Recruitment Incentive Package up to the value of $10,000 (conditions apply and may be pro-rata’d to your contracted or ordinary working hours, these payments are paid on a fortnightly basis) under theNSW Health Rural Health Workforce Incentive Scheme.

This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.

What you’ll be doing

The Senior Allied Health Clinician – Disability and Complex Care provides advanced clinical assessment, intervention, consultation, care coordination and service development for people with disability and complex health needs across Far West Local Health District.

• Support the Hospital Social Work team and multidisciplinary teams in the management of complex patients, including discharge planning, guardianship matters, accommodation challenges, psychosocial complexity, NDIS interface issues and transitions of care.
• Provide expert advice, consultation and capacity building to clinicians regarding the assessment and management of people with disability accessing health services.
• Facilitate coordination between NSW Health services, disability providers and community agencies to support safe admissions, discharge planning and transitions of care for people with disability and complex needs. Develop and maintain strong stakeholder relationships to improve integration between health, disability and community services and promote positive patient outcomes.
• Identify and respond to barriers impacting access to health services for people with disability and work collaboratively with stakeholders to develop and implement solutions.
