Job summary

Purple House in Perth, WA, is seeking a Part-time Permanent Associate to join the Social Support Worker team supporting dialysis patients and their families. You will provide wrap-around care, practical assistance, and collaborate with Elder Care and Clinical Services to help clients live their best life.

The role emphasizes Indigenous community control, compassionate communication, and advocacy across health, housing and financial needs.

Qualifications

• Indigenous community–controlled approach is essential.
• Courteous, respectful communication with stakeholders.
• Patient-centered focus and goal of getting people home on Country.
• Compassion, empathy and active listening.
• Ability to think on your feet to negotiate and solve problems.
• Conflict management considered and thoughtful.
• Current National Police Check – less than 6 months old
• First Aid & CPR or ability to obtain

Responsibilities

• Provide practical support for clients with complex needs.
• Collaborate with Elder Care and Clinical teams to support holistic care.
• Encourage and participate in client social activities.
• Foster strong relationships with external stakeholders.
• Advocate for clients to access health, housing, financial and cultural services.
• Document activities in daily work sheets.
• Contribute to organisational planning, implementation and evaluation.
• Promote a safe work environment and comply with WHS guidelines.
