Job summary

Maari Ma Health is seeking a Clinical Nurse Consultant in Diabetes Education to lead education and clinical consulting within a GP-led team. The role involves outreach across communities in the Broken Hill region with travel and possible overnight stays.

Key requirements include AHPRA nursing registration, a Graduate Certificate or Diploma in Diabetes Education, and credentialing with ADEA or eligibility within 12 months.

Qualifications

• Unconditional registration with AHPRA as a registered nurse.
• Graduate Certificate or Diploma of Diabetes Education.
• Credentialed Diabetes Educator with ADEA (or eligibility to obtain credentialling within 12 months)
• Demonstrated experience in diabetes and chronic disease management
• Strong communication and interpersonal skills.

Responsibilities

• Deliver high-quality, individualised diabetes care and education for patients and families at risk or living with diabetes and related conditions.
• Collaborate with multidisciplinary health teams to plan, coordinate, and implement effective chronic disease management.
• Provide clinical leadership and guidance to Aboriginal Health Practitioners, Primary Health Nurses and allied health professionals.
• Support early intervention through patient referrals, evidence-based practice, and promotion of best-practice clinical standards.

About the role

An interesting and challenging balance of leadership, education and clinical consulting within a GP-led multidisciplinary team; an opportunity to work with front line clinicians to build capacity and expertise in our region.

This position is located in Broken Hill, however delivery of outreach services to communities of the region with travel and occasional overnight stays required.

To be successful in this role you will have;

• Unconditional registration with AHPRA as a registered nurse
• Graduate Certificate or Diploma of Diabetes Education through a recognised Australian University.
• Credentialed Diabetes Educator with ADEA (Australian Diabetes Educators Association) (or eligibility to obtain credentialling within 12 months)
• Demonstrated experience in diabetes and chronic disease management
• Strong communication and interpersonal skills

Key Responsibilities

Salary up to $143,532gross per annumplus super
