Job summary

Belong Blue Mountains Incorporated – Blaxland NSW is inviting applications for a community-focused role supporting people living with dementia and their carers. You will build supportive relationships, help navigate aged care services, and coordinate referrals with health professionals and community services in a busy, community-based setting.

You will lead dementia-friendly activities, run carer support groups, and contribute to program evaluation.

Qualifications

• Relevant tertiary qualification in community services, health, allied health, social work, ageing/gerontology or related discipline, and/or substantial experience.
• Demonstrated experience supporting people living with dementia and their carers.
• Sound knowledge of dementia, ageing, person-centered practice and aged care services.
• Experience in care coordination, group facilitation, education or program delivery.

This is a varied, community-based role providing person-centered wellness navigation, practical support, education and service coordination for people living with dementia and their carers.

You will facilitate dementia activity and wellness programs and carer support groups, connect individuals and families with appropriate aged care, health and community services, and help build confidence, independence, wellbeing and social connection.

Working within your professional qualifications and scope of practice, you will identify strengths, goals, risks and support needs, build strong referral pathways and contribute to Belong Blue Mountains' broader commitment to an inclusive and dementia-friendly community.

Key Responsibilities

• Build trusted, person-centered relationships with people living with dementia and their carers
• Support clients and carers to identify wellbeing goals, support needs and appropriate service options
• Assist people to navigate My Aged Care, Support at Home, CHSP, respite and other relevant services
• Coordinate referrals and connections with health professionals, aged care providers and community services
• Provide practical information, education and support to carers, including strategies that promote wellbeing and resilience
• Plan, coordinate and facilitate dementia-friendly activity and wellness programs
• Establish and facilitate carer support groups, information sessions and community education activities
• Build effective relationships and referral pathways with local health, aged care and community organisations
• Identify and respond appropriately to wellbeing, safeguarding and other risks, escalating clinical or high-risk matters as required
• Maintain accurate client, program, participation and outcome records in accordance with organisational and funding requirements
• Contribute to program evaluation, service improvement and dementia-friendly community initiatives
• Work collaboratively with clients, carers, colleagues, volunteers and service partners to support positive outcomes

About You

You will be a compassionate, practical and community-minded professional who enjoys building relationships and supporting people living with dementia and their carers.

You will be confident working independently in the community, facilitating groups and programs, navigating service systems and recognising when clinical, safeguarding or higher-risk matters need to be referred or escalated.

Applicants must demonstrate:

• Relevant tertiary qualification in community services, health, allied health, social work, ageing/gerontology or a related discipline, and/or substantial relevant experience.
• Demonstrated experience supporting older people, including people living with dementia and their families or carers.
• Sound knowledge of dementia, ageing, person-centered practice and the aged care and community services environment.
• Experience in service navigation, care coordination, group facilitation, education and/or community-based program delivery.
• Strong communication and relationship-building skills, with the ability to work effectively with clients, carers, service providers and community stakeholders.
• Demonstrated ability to work autonomously, exercise sound judgement, manage risk and safeguarding matters, and maintain accurate and timely documentation.
• Experience with My Aged Care, Support at Home, CHSP and dementia/carer support services.
• Additional training or experience in dementia care, carer education, community development or health promotion.
• Current NSW Driver Licence.
• NDIS Worker Screening and any other screening requirements relevant to the role.
• Professional registration or accreditation where required by the successful candidate’s discipline.
